Diazepam for Muscle Spasm and Back Pain — How It Works
Diazepam is a benzodiazepine first and a muscle relaxant second — but for acute spasm of the back or neck, it remains one of the most useful tools UK GPs prescribe.
Why it works for spasm
Diazepam acts on GABA receptors in the spinal cord as well as the brain, reducing the reflex arc that locks muscles in spasm. NSAIDs (ibuprofen, naproxen) reduce pain and inflammation but don’t unlock the spasm itself.
Typical short course
2mg–5mg three times a day for 3–5 days, alongside an NSAID for pain. Stop as soon as the spasm releases — usually within 48–72 hours.
Why not longer
Tolerance starts to build at the 2-week mark. For chronic back pain, physiotherapy and core strengthening do more than any pill.
